All ad copy must be scan ready. If you need a design for your ad please add $5.00. 
Ads that are hand-drawn must be done in BLACK marker. 
All ad copy and payment must be turned in to Casa Dance Studio by May 31st 2014. 

When completing the order form please PRINT. Enclose form, payment and ad copy in an envelope. DO NOT staple of fold ad copy that needs to be scanned. 

Checks should be made out to Casa Dance Studio.
